Pre-authorisation & Documentation Audit
Find documentation gaps before submission.
The problem
Small omissions can become expensive delays when they are discovered after submission.
What the package includes
- Pre-authorisation workflow review
- Documentation checklist review
- Record completeness sampling
- Exception identification
- Corrective action plan
- Team briefing and handover
How an engagement works
First, scope the workflow and agree access. Next, review the relevant process and document exceptions. Then, assign corrective actions and support the agreed submission or follow-up steps. Finally, review progress with your nominated lead.
Scope and fees
Fees are quoted against an agreed scope. Request a written proposal covering deliverables, exclusions, responsibilities, reporting, the fee basis and review dates before committing. We agree whether the work is a defined project or an ongoing service before it starts.
Who this is for
Facilities seeking a more consistent claims process and clear ownership of outstanding actions. The review should include your finance lead and the people responsible for documentation.
Check the handovers behind the paperwork
Documentation quality depends on the process that produces a record. A completeness audit looks at where information is captured, who confirms it and what happens when something is missing. The aim is to identify administrative gaps early, without replacing clinical review or making a payer decision.
We begin by agreeing the workflow, sample and criteria for the review. Your facility identifies the applicable payer instructions and the people authorised to clarify records. We then compare the selected records and handovers against that agreed checklist and record exceptions consistently.
Review before the point of submission
Pre-authorisation and documentation checks can involve different teams and deadlines. A useful workflow makes those responsibilities visible. It identifies who confirms a requirement, who follows up a missing item and who decides whether the file is ready for the next step.
The audit does not assume that every missing item has the same consequence. Findings should distinguish incomplete administrative information, unclear ownership, evidence requiring clarification and system issues. Your authorised team decides how each item is corrected using the underlying record and current payer instructions.
From findings to a usable checklist
The output is a prioritised action plan rather than a list of faults without owners. Each recommendation should have a responsible person, a review date and a practical way to check whether the change is working. A short team briefing helps staff understand the revised handover and escalation arrangements.
A sample review has limits. It describes the records and process reviewed, rather than certifying every future claim. We explain those boundaries in the engagement scope and identify questions that require a wider review. No audit guarantees authorisation, claim approval or payment.
For an initial discussion, describe your facility, the part of the process causing concern and the scale of the work. Do not upload clinical records here. Any operational review starts only after confidentiality, access and data-handling arrangements are agreed. Explore our healthcare services or contact Alata Vision for a scoped proposal.
